The following information is requested to allow your shipmates to honor you upon your departure on Eternal Patrol. 

This Information will be held by the Base Chaplain and Base Commander, and only shared with your approval.

FYI:   Dates need not be exact (ie - April 1972, or Spring 1972 are accurate enough for this form)
